Travel Well Giveaway Rules

NO PURCHASE OR PAYMENT NECESSARY TO ENTER OR WIN. A PURCHASE OR PAYMENT WILL NOT INCREASE YOUR CHANCES OF WINNING. VOID WHERE PROHIBITED BY LAW.

1. SPONSOR: The Revive MD Travel Well Giveaway (“Sweepstakes”) is sponsored by Revive MD Supplement Company LLC, 760 NW Enterprise Dr., Port St. Lucie, Florida 34986 (“Sponsor”).

2. ELIGIBILITY: The Sweepstakes is open only to legal residents of the fifty United States and the District of Columbia who are 21 years of age or older at the time of entry. Employees, officers, and directors of Sponsor, its subsidiaries, affiliates, advertising and promotion agencies, and any other entity involved in the development, administration, promotion, or implementation of the Sweepstakes (collectively, the “Sweepstakes Entities”), and the immediate family members (spouse, parents, siblings, and children, regardless of where they live) and persons living in the same household (whether or not related) of such individuals are not eligible to enter or win. The Sweepstakes is void where prohibited by law.

3. SWEEPSTAKES PERIOD: The Sweepstakes begins at 12:01 AM Eastern Time (“ET”) on November 15, 2024, and ends at 11:59 PM ET on December 2, 2024 (the “Sweepstakes Period”).

4. HOW TO ENTER:

• Each dollar spent on Revive MD products (exclusive of taxes, shipping, or other fees) during the Sweepstakes Period will count as an entry into the Sweepstakes
• To obtain an entry without making a purchase, please send correspondence requesting entry into the Revive MD Travel Well Giveaway and include your name, mailing address, telephone number, and e-mail address. Please mail your entry to: Revive MD, Attn: Hannah Rowston, 760 NW Enterprise Drive, Port St. Lucie, FL 34986. Your mail-in entry must be postmarked during the Sweepstakes Period. Each mail-in submission will receive 10 entries to the Sweepstakes. Limit one mail-in submission per person. 

5. SELECTION OF WINNERS: Winners will be contacted by the Sponsor at the end of the Sweepstakes Period. Winners will be selected at random from among all eligible entries received and selection will be after the conclusion of the Sweepstakes Period. Winners will be contacted via phone or email on December 5, 2024. In the event that Sponsor cannot reach the Winners on December 5, 2024, Sponsor will select at random replacement winners and contact them beginning on December 6, 2024. Entrants are responsible for providing Sponsor with valid phone numbers and e-mail addresses in order to receive notification of winning the Sweepstakes. Each potential winner will be required to complete, sign, and return an Affidavit of Eligibility, a Liability Release, and where lawful a Publicity Release (collectively, “Prize Claim Documents”) within five (5) days of attempted notification or the prize may be forfeited.

 

6. PRIZES; APPROXIMATE RETAIL VALUE; ODDS:

• Duffel bag carryon filled with Revive MD products (approximate value of $500) each

Odds of winning depend on the number of entries received. All prizes awarded are “as is” and contain no warranty or guarantee, whether express or implied. Sponsor reserves the right, at Sponsor’s sole discretion, to substitute an alternative prize, in whole or in part, of equal or greater value, in whole or in part, if a prize cannot be awarded, in whole or in part, for any reason.

7. GENERAL CONDITIONS: By entering, participants agree to be bound by these Official Rules and the decisions of the Sponsor, which are final and binding in all respects. Sponsor reserves the right to disqualify any participant it finds to be tampering with the entry process or to be acting in violation of these Official Rules.

8. RELEASE AND LIMITATIONS OF LIABILITY: By participating in the Sweepstakes, participants agree to release and hold harmless the Sweepstakes Entities from any and all liability for any injuries, losses, or damages of any kind arising from or in connection with participation in the Sweepstakes or the acceptance, possession, use, or misuse of any prize.

9. PUBLICITY: Except where prohibited, participation in the Sweepstakes constitutes the winner’s consent to Sponsor’s and its agents' use of the winner’s name, likeness, photograph, voice, opinions, and/or hometown and state for promotional purposes in any media, worldwide, without further payment or consideration.

10. GOVERNING LAW: This Sweepstakes and the rights and obligations of the Sponsor and participants will be governed by and construed in accordance with the laws of the State of Florida without giving effect to any choice of law or conflict of law rules. Claims arising out of or connected with the Sweepstakes, shall be resolved solely and exclusively in a federal or state court located in St. Lucie County, Florida. Any and all Claims, judgments, and awards shall be limited to actual damages of no more than $100, including costs associated with entering this Sweepstakes, but in no event attorneys’ fees. Sponsor shall not be liable for special indirect consequential damages arising out of or connected with the Sweepstakes.

11. PRIVACY: Any personal information collected in connection with the Sweepstakes will be used by Sponsor solely to administer the Sweepstakes and will be subject to Sponsor’s privacy policy.

12. WINNER’S LIST: To request the name of the winners, send a self-addressed, stamped envelope to: Revive MD, Attn: Hannah Rowston, 760 NW Enterprise Dr., Port St. Lucie, Florida 34986. Requests must be received by January 31, 2025.
